Improving Fuel Economy as per Lexus

Lexus suggests using premium fuel vs. regular. Does this in fact improve mileage? If you calculate the increased cost per gallon and it improves over 2 mpg it is worth it, but does it really improve mpg? Ironically, they suggest driving without roof racks which are std on their RX400h. Any thoughts?
If one doesn't have a long stretch of road how can you use cruise control? Thoughts?
